I understand that there are certain inherent risks and dangers in attending and/or participating in any activity and/or events organized by Tsunami Sports & Recreation which include, but are not limited to, illness, physical harm, loss, bodily injury, death or property damage. I assume all of the risks that may or can arise from participating in the activity and event, including unavailability of emergency care and those risks described in the participant conduct policy.
I hereby consent for my child to participate in the activities provided by Tsunami Sports & Recreation. I confirm that all my contact information is correct and I am able to give parental consent for my child to participate in the event. I acknowledge that Tsunami Sports & Recreation is not responsible for providing adult supervision for my child.
To the fullest extent permitted by law, I agree to indemnify and hold harmless Tsunami Sports & Recreation and its organizers and each respective member and representatives from any and all responsibility, claims, causes of actions and suits. I acknowledge that the organization is not held responsible for any personal lost or damaged items any time before, during, or after the event.
I understand and expressly agree that this agreement is intended to be inclusive as permitted by the law and regulations of Ontario for which it is used and that if any portion of this agreement is held invalid, it is agreed that it shall continue in full effect.
